REPORTS SUBMISSION

SUBMISSION OF REPORTS BY LICENSED PERSONS

Licensed persons who use, store or manufacture ionizing radiation sources, according to Procedure on submitting report on radiation situation by licensee using radioactive materials or devices containing radioactive materials or radiation generators (approved under the ANRA order No 317-N as of 12 October 2010) are required to submit reports on the state of radiation safety to ANRA.

Reports on radiation safety are

The reporting frequency is specified in Table N1. Periodic reports are submitted after the end of the reporting period until the second quarter of the following year.

Operational report (message) is submitted within 24 hours in case of accident/incident occurred during implementation of activity.

Summary report is submitted within one month after the end of investigation of causes of accident/incident.

PERIODIC REPORT

Periodic report should include the following information:

  • Details of licensee submitting a report

  • Reporting period

  • Licenses,

  • Radiation safety officer details,

  • Person prepared periodic report,

  • Inspections,

  • Inventory of sources,

  • Radiometric measurements* results,

  • Dosimetric measurements results,

  • Equipment operability,

  • Personnel training,

  • Incidents/accidents,

  • Number of patients diagnosed/treated with medical exposure.

OPERATIONAL REPORT

Operational reports (messages) should contain the following information:

  • Details of a licensee submitting report.

  • In case of a legal entity: name, registration address, place(s) where activity is implemented and address (addresses), contact details (telephone, e-mail address).

  • In case of an individual entrepreneur: full name, address of residence, place(s) where activity is implemented and address(s), contact information (telephone, e-mail address).

  • Number of licenses granted by ANRA for implementation of activity, validity period,

  • Place where incident/accident occurred and time,

  • Nature of incident/accident (overexposure of personnel/patients, depressurization of source, loss, theft, etc.),

  • Initial cause of incident,

  • Initial number of personnel, patients and (or) individuals exposed as a result of incident/accident and exposure doses,

  • Radiation measurement data (gamma radiation, radioactive contamination) for radiation situation assessment,

  • A brief description of actions on elimination or mitigation of incident or its consequences, as well as actions to protect personnel and others.

  • Details of a licensee submitting report.

  • In case of a legal entity: name, registration address, place(s) where activity is implemented and address (addresses), contact details (telephone, e-mail address).

  • In case of an individual entrepreneur: full name, address of residence, place(s) where activity is implemented and address(s), contact information (telephone, e-mail address).

  • Number of licenses granted by ANRA for implementation of activity, validity period,

  • Place where incident/accident occurred and time,

  • Nature of incident/accident (overexposure of personnel/patients, depressurization of source, loss, theft, etc.),

  • Initial cause of incident,

  • Time when incident was registered and eliminated,

  • Initial number of personnel, patients and (or) individuals exposed as a result of incident/accident and exposure doses,

  • Detailed description of measures at incident/accident elimination or mitigation, as well as actions to protect personnel and others,

  • Radiation measurement data (gamma radiation, radioactive contamination) for radiation situation assessment,

  • Root cause (s) of incident/accident,

  • Lessons learned, measures taken to prevent re-occurrence of incident/accident.
